ABSTRACT

Objective to investigate the level of platelet leukocyte aggregates in patients with acute cerebral infarction and their short term prognosis.Methods 105 patients with acute cerebral infarction onset within 24 hours were selected continuously,then platelet leukocyte aggregates including neutrophil aggregates (PNA) and platelet monocyte aggregates (PMA) and platelet lymphocyte aggregates (PlyA) were detected by flow cytometry within 24 hours of admission and the incidence of 14 days.modified Rankin Scale(mRS) was performed at 14 days of onset,as a prognostic indicator,and the mRS score was good at 3.The score >3 mRS was divided into poor prognosis.The level of platelet leukocyte aggregates was detected in 50 healthy subjects.Results (1) The platelet leukocyte aggregates in patients with acute cerebral infarction group were significantly higher than that of the healthy group,which was statistically significant (P3 score comparison,the difference of white blood cell aggregates was statistically significant(P<0.05).Conclusion leukocyte aggregates could be used as an index of short-term prognosis in patients with acute cerebral infarction.

ABSTRACT

Objective To investigate whether blood lipid control can delay the progression of asymptomatic gallstones and reduce the risk of cholecystectomy in patients with gallstones and hyperlipidemia,as well as the influence of hyperlipidemia on postoperative pain after cholecystectomy.Methods A total of 153 patients with asymptomatic gallstones and hyperlipidemia who underwent physical examination from February 2013 to February 2015 were enrolled and randomly divided into experimental group with 72 patients and control group with 81 patients.The patients in the experimental group were given blood lipid control via diet,exercise,and drugs,and according to fasting triglyceride (TG) and total cholesterol (TC) after 3 months,these patients were further divided into normal blood lipid group with 47 patients and abnormal blood lipid group with 25 patients.All the patients were followed up for 2 years with an interval of 3 months.The surgical indications for laparoscopic cholecystectomy were persistent pain in the gallbladder or more than 3 times of gallbladder discomfort within the past one month.A subgroup analysis was performed based on the nunber and size of gallstones to evaluate the risk of cholecystectomy.A numerical pain scale was used to assess the improvement in pain during hospitalization and at 3 and 6 months after surgery.The t-test was used for comparison of continuous data between two groups;a one-way analysis of variance was used for comparison between three groups,and the Bonferroni test was used for further comparison between any two groups.The chi-square test was used for comparison of categorical data between groups.Results The normal blood lipid group had a significantly lower rate of cholecystectomy than the abnormal blood lipid group and the control group (23.4% vs 68.8%/70.4%,x2 =27.72,P <0.O1).The patients in the normal blood lipid group had moderate pain during hospitalization,while those in the abnormal blood lipid group and the control group had severe pain,and there was a significant difference in pain score between the normal blood lipid group and the abnormal blood lipid group/control group (P < 0.05).Among the patients with a single gallstone,multiple gallstones,≥1 cm stones,or < 1 cm stones in the abnormal blood lipid group,61% (11/18),86% (6/ 7),88% (7/8),and 59% (10/17) underwent cholecystectomy,while in the normal blood lipid group,21% (6/29),28% (5/18),35% (6/17),and 17% (5/30) underwent cholecystectomy,and there were significant differences in the proportion of patients with different types of gallstones who underwent cholecystectomy between the two groups (x2 =7.86,6.87,6.12,and 8.77,all P < 0.05).Conclusion In patients with gallstones and hyperlipidemia,effective blood lipid control can significantly delay disease progression and reduce the risk of cholecystectomy for different types of gallstones and may alleviate postoperative pain.

ABSTRACT

Objective To approach the changes of cytosol phospholipase A2α(cPLA2α)and nitric oxide (NO)in patients with chronic obstructive pulmonary disease(COPD)and its significance. Methods One hundred patients with COPD admitted into Department of Critical Care Medicine of Affiliated Wuqing Traditional Chinese Medicine(TCM)Hospital of Tianjin University of TCM were enrolled,and according to the COPD severity grading standards,they were divided into mild group(25 cases),moderate group(25 cases),severe group(26 cases) and extremely severe group(24 cases);simultaneously,90 cases with normal pulmonary function who had taken health examination were chosen and assigned to the healthy control group. The cPLA2α level was detected by enzyme linked immunosorbent assay(ELISA),the level of uric acid(UA),total cholesterol(TC),triacylglycerol (TG)were detected by enzymatic method,and serum NO metabolites(NOx)level was detected by nitrate reductase method. Results Compared with the healthy control group,the serum levels of cPLA2α and UA in patients with different severity of COPD were significantly increased;along with the increase of patient's COPD grade of severity,the cPLA2α,UA levels were gradually increased,while NOx level was gradually decreased in mild, moderate, severe, extremely severe groups〔cPLA2α(ng/L):125.60±8.17, 155.20±6.42, 190.20±9.32, 255.80±11.28 vs. 88.50±7.99;UA(μmol/L):381.23±32.22,434.95±87.71,464.81±52.65,487.45±82.61 vs. 241.95±52.33;NOx(μmol/L):59.90±17.52,45.60±6.17,38.20±4.08,25.70±3.04 vs. 74.90±18.31,all P0.05). The cPLA2αand NOx levels presented significant negative correlation(rs=-0.798,P=0.013). Conclusion The combined examination of blood cPLA2αand serum NOx levels can evaluate the severity degree of COPD patients,and cPLA2αcan be used as a new target index for COPD grading.

ABSTRACT

@#The patients with cor pulmonale in release stage have been treated and cared with many methods.The results showed that there are obvious recuperatio of respiratory symptoms and lung function and evident strength of the immune function of the body and movement of diaphragm in the patients of treatment group with cor pulmonale compared with control group.The studies suggested that the rehabilitation treatment and care plays an important role in treating patients with cor pulmonale,particularly postponing and prevention acute attack.

ABSTRACT

Nine adult rabbits, perfused with 10% formalin, were used in this experiment. Five of them were mounted on the stereotaxic apparatus according to the Sawyer's atlas. Brains were removed and made serial sections including thalamus and hippocampus. The neocortices of the other four rabbits were removed and the hippocampal formations were exposed. Serial sections of the hippocampus were made and stained alternately with Nissl's method and the silver impregnation method of Glees. The results could be summarized as follows.1. The hippocampus measured 28.00-36.00 mm in length, 6.00-8.50 mm in breadth and 2.68-3.39 mm in thickness. The total numbers of the pyramidal cells (CA_1-CA_4), the granular cells in the dentate gyrus and the basket cells were 7.36?10~6, 4.93?10~6, 2.40?10~5 respectively. The ratio of the total numbers among the pyramidal, granular and the basket cells was 30:20:1 approximately. The total number of the CA_1 pyramidal neurones was 4.11?10~6, and that of the CA_3 was 2.48?10~6. The ratio of the numbers of the CA_1 and CA_3 neurones was 1.66:1.2. The arrangements of the granular cells in the dentate gyrus and the apical dentrites of the CA_1 neurones were rather regular and possessed a typical array-like structure.3. The total numbers of the fibers in the column fornix (FN)and the mamillothalamic tract (MT) were 83700 and 58740 respectively. MT:FN=1:1.42. The total number of the neurones in the mamillary nucleus was 113500, and that of the anterior thalamic nucleus was 165600. The ratio of the neurones between these two nuclei was 1:1.46.According to the results mentioned above, the functional significance of the characteristic structure of the hippocampus was also discussed.
